1. Introduction

Welcome to Wasatch Wellness Chiropractic. These Terms and Conditions outline the rules and regulations for our monthly membership plans. By enrolling in a membership plan, you agree to comply with these terms.

2. Membership Plans

We offer three membership plans:

Once a Month: One chiropractic session per month.

Twice a Month: Two chiropractic sessions per month.

Family Plan: Designed for families, offering multiple sessions per month. Specific details and limits of this plan will be provided at the time of enrollment.

3. Membership Duration and Renewal

Memberships are for a term of 12 months.Membership benefits roll over for one month if unused. Unused benefits do not accumulate beyond one month.Memberships will automatically stop after 12 months unless you choose to continue.You will have the option to renew your membership at the end of the 12-month term.

4. Billing and Payment

Billing occurs on the same day of each month corresponding to your purchase date. You have the option to choose between monthly billing or a one-time payment for six months of membership.

5. Cancellation and Refunds

Memberships can be canceled at any time. However, there are no refunds for unused portions of the membership.If you cancel your membership, any unused benefits will be forfeited.

6. Membership Changes

You may upgrade or downgrade your membership plan at any time. Changes will take effect in the next billing cycle.Any changes to membership plans will be subject to the current pricing at the time of the change.

7. Use of Services

Membership benefits are personal and non-transferable. Family Plan benefits are restricted to immediate family members as defined at the time of enrollment.

8. Termination by Wasatch Wellness Chiropractic

We reserve the right to terminate your membership if you violate any of the terms and conditions.In the event of termination by us, you will be refunded a pro-rata portion of any prepaid membership fees.

9. Liability

Wasatch Wellness Chiropractic is not responsible for any injuries or health conditions that occur during or after chiropractic sessions. It is your responsibility to inform the chiropractor of any pre-existing conditions or concerns.

10. Changes to Terms and Conditions

Wasatch Wellness Chiropractic reserves the right to modify these terms and conditions at any time. Members will be notified of any changes in advance.

11. Governing Law

These terms and conditions are governed by and construed in accordance with the laws of the state where Wasatch Wellness Chiropractic operates.
